The Health Care Directive helps your doctors determine when life-supporting measures should continue or stop. If your wish is to not use life-sustaining measures, this can be communicated to your health care provider by your Health Care Directive agent.
The person you appoint as your Health Care Directive agent has no authority to make decisions for you until you are unable to make those decisions yourself. When you become incapacitated, this person must make decisions that are consistent with any instructions you have written in the Health Care Directive. |

You can appoint almost any adult to be your Health Care Directive agent. You can choose a member of your family such as your spouse or an adult child, a friend, or someone else you trust. You can also appoint one or more people in case the person you select as your Health Care Directive agent is unavailable or unwilling to make a decision.
